Septum piercing: pain, healing, and hiding it for work

Pain
4/10
Healing
6–8 weeks
Starter jewelry
16g titanium clicker, circular barbell, or retainer

The septum sits between your nostrils. Done right, it goes through a thin column of soft tissue called the 'sweet spot' — not through cartilage. Done wrong (through the cartilage wall), it hurts a lot more and takes much longer to heal.

Why your eyes water

The trigeminal nerve runs through your nose. When a needle crosses it, your body fires a reflex that floods your eyes. It's not pain — it's plumbing. The actual piercing sensation is a quick pinch.

Healing

Surprisingly fast for a face piercing — 6–8 weeks for the outside, 3–4 months for the inside to fully settle. Clean with sterile saline twice a day and resist the urge to flip the jewelry up and down constantly.

Hiding it

Once healed, swap the starter jewelry for a 'septum retainer' — a small horseshoe-shaped piece you can flip up inside your nostrils, making it nearly invisible. Most piercers stock them in titanium.

The smell

Around weeks 3–6, many people notice a strange smell from their septum jewelry. It's normal — a mix of sebum, lymph, and dead skin cells. A daily saline rinse and a gentle wipe with a clean cotton swab clears it.

FAQ

How long does a septum piercing take to heal?

6 to 8 weeks for the outside, 3–4 months for the inside to fully settle. Faster than most face piercings because it goes through soft tissue, not cartilage.

How much does a septum piercing cost?

Expect $50–$90 at a reputable US studio for the piercing, plus $30–$80 for a titanium clicker or circular barbell. Total: $80–$170.

Does a septum piercing hurt?

Most people rate it 4/10. The pain is a brief pinch — the surprise is that your eyes water uncontrollably from the trigeminal nerve reflex. It's plumbing, not pain.

What gauge is a septum piercing?

Typically 16g or 14g. Anything thinner risks the jewelry migrating or tearing the sweet spot.

When can I change my septum piercing?

Wait at least 6–8 weeks and only after a piercer confirms it's healed. Changing too early causes irritation and that distinctive 'septum smell.'

How do I clean a septum piercing?

Sterile saline spray twice daily, with the head tilted back so it runs through the channel. Resist the urge to flip the jewelry up and down constantly while healing.

Do septum piercings close?

An unhealed septum (under 3 months) can close in hours if jewelry is removed. A fully healed septum left empty closes more slowly but can still shrink within days or weeks.

Why does my septum piercing smell?

Around weeks 3–6 it's normal — a mix of sebum, lymph, and trapped skin cells. Daily saline rinses and a gentle cotton-swab wipe of the jewelry clears it.

Can a septum reject?

Almost never when placed correctly in the sweet spot. Off-center or cartilage piercings can migrate.

Does a stretched septum close back?

If stretched slowly and carefully, yes — usually within a few months of removing jewelry. Aggressive stretching can leave a permanent split.
